Scope. This policy covers information collected through this website, such as the details you submit in a contact or assessment form. It does not cover protected health information (PHI) handled in our role administering health benefits, which is governed by our HIPAA Notice of Privacy Practices. Where information is subject to HIPAA, HIPAA controls.

When you submit a form on this site, we collect what you choose to provide, which may include your name, email address, phone number, employer or agency name, job details (such as number of employees or renewal date), practice details (such as NPI or specialty), and the contents of your message.

Information collected automatically

Our hosting provider and web server may automatically record standard technical information, such as IP address, browser type, referring page, and the pages you request, in server logs. This is used to operate and secure the site.

Cookies and analytics

This website does not currently set advertising or tracking cookies and does not run third-party analytics. If that changes, this policy will be updated and, where required, we will ask for your consent.

How we share your information

Children's privacy

This website is intended for businesses and adults. We do not knowingly collect personal information from children under 16 through this site.
