What to include

A complete claim processes without back-and-forth. Before you send, make sure you've included:

  • Member ID exactly as shown on the Bedrock ID card
  • Correct dates of service
  • Accurate procedure (CPT) and diagnosis codes
  • Provider name, Tax ID, and NPI
  • Any prior authorization reference, where applicable
